Evaluating your clinical sites regularly is important for providing high quality experiential education, and the custom site assessment form is here to make it easier for you!

The site assessment form helps schools to collect detailed feedback about clinical sites. This information can be used to evaluate site performance, quality of experiential education, planning future placements, and get important insights for your accreditation.

How to access, setup, and use the form:

1. Navigate to: Sites > Setup > Form Setup
2. Create a new form: Use the 'Create New Form' button to add questions and sections using different elements based on your requirements. If you already use a form today, you can also copy from the existing configured forms and edit the questions to create a new form and save effort
3. Fill out the form: Go to the 'Assessments' section for a location or site and click on the 'Add assessment' button to start filling out the form
4. Review Responses: Use the 'Comprehensive Assessment Report' to analyze the data collected to make decisions
