Once your students begin submitting patient logs, you’ll be able to review their logs and mark any if they need edits.
- Select Learning Activities from the left menu or dashboard.
- Select Patient Logs for the desired course offering.
- You can also find the course or rotation you’d like to review using the Learning Activity Groups, simply select your desired course or rotation.
- You’ll be taken to the Patient Logs page, which will display all placed students and their current log count, a breakdown by status, and recent activity details.
- Click on any of the status counts to begin reviewing patient logs.
- A drawer will open. You’ll have a tally at the top indicating how many logs are in each status.
- You can easily approve all logs or multiple logs using the open checkboxes.
- To review an individual log, click on the LOG ID.
- The log will open, you can then review the form. If the form looks good, you can:
- Approve the log: will inform the student the log has been reviewed and will not allow them to make further changes.

- If the form needs additional information or changes, provide feedback, and select Needs Attention.
- You can also approve logs in bulk for a selected student!
- To view a statistics report on a single student, click on the graph icon on the Patient Logs main page.
- A drawer will open, and you can view information by setting and rotation.
- Please note, the statistics report is updated four times a day. Data entered may not be reflected instantly.

Patient Log Reports
Patient log reports in Exxat provide a comprehensive overview of patient data, including demographics, clinical sessions, treatment progress, outcomes, and documentation. These reports are invaluable for quality assurance, research, and regulatory compliance. Exxat Prism offers various report types, such as individual patient reports, group reports, and statistical reports. Additionally, users can customize reports to meet specific needs, such as filtering data or exporting reports. Below is the list of all the available reports.
Navigating to Reports
- Select 'Learning Activities' from your left menu or dashboard.
- Click on 'Reports' on the top left hand of the screen.
- You will see a list of all the reports available for Learning Activities in your program with a short description of the report.
Reports
- Aggregate Log Count Report is a metric that measures the total number of patient logs submitted by a student across their clinical rotations. It provides insight into a student's clinical activity and engagement but does not assess the quality of the documentation. This metric can be used to track student progress, evaluate workload, identify areas for improvement, and assess the effectiveness of the clinical education program.
A. After clicking on the report name, click on 'Select Course Offerings' to get a list of all Course Offerings in your program.
B. You will see a list of all course offerings available in your program. You can search for specific courses using the Course Number, Course Name, Academic Year, Professional Year, Term, Cohort or Campus.
C. After selecting the course offerings, the page will show you a list of all the selected courses. If you wish to get more granular results, you can apply the filters for on 'Rotation', 'Settings' or 'Status'. Once the selections have been made you can click on the 'Generate Report' button. - Aggregate Raw Data Report is a comprehensive dataset that includes all the unprocessed information collected from patient logs submitted by students during their clinical rotations. This raw data offers a detailed view of each student's clinical activity, allowing for in-depth analysis of their performance, behavior, and patterns. It can be used for various purposes, such as research, quality improvement, benchmarking, and individual student analysis.

Once the selections have been made you can click on the 'Generate Report' button. - Patient Log Report by Rotation provides a detailed analysis of a student's performance within a specific clinical placement. It filters patient log data to focus on the student's interactions with a particular preceptor, offering insights into treatment time, outcomes, and comparisons with other students in the same rotation. (Please note: This report gets updated every day at 12AM, 6AM, 12PM & 6PM Eastern Time)

If you wish to get more granular results, you can apply the filters for on 'Rotation', 'Settings' or 'Status'. Once the selections have been made you can click on the 'View Statistics' button.
Note: Statistics can be view either by Settings or Rotation.
D. In the Rotation view you will first select the student and then see information such as Rotation Details, Total number of Patient Logs, Age Group of Patients, Environment of Care, If any collaborative care was provided & Any Diagnosis on the basis of Student Rotation.
E. In the Setting view also you will first select the student and then see information such as Rotation Details, Total number of Patient Logs, Age Group of Patients, Environment of Care, if any collaborative care was provided & Any Diagnosis on the basis of each setting.
