Viewing Location Evaluations

Within Prism, you can set evaluations to be shared with students.

For our PT-PTA programs, the Student Evaluation of Site automatically comes with this feature enabled.

For all programs: if you have a form you’d like to be shared with students, contact v4support@exxat.com to have this feature enabled for existing forms, or if you’d like to create a new form with this feature.

1. Select Sites from the left-menu or dashboard.
   
   
   
2. Select Locations from the ribbon at the top of the page.
   
   
   
3.  Find and select your desired Location.
   
   
   
4. Click on the Location Evaluations tab from the left panel.
   
   
   
5. Click View for any evaluation you’d like to access.
   
   
   
6.  The form will load for you to view.
