During a placement, if your program uses standard or custom forms that need to be completed by Sites, you can distribute them when needed!
The Form(s) table provides the details such as Form name, No of placements, Rotation, Distribution Status, Submission Status and Action to be taken.
The distribution status column shows the number of distributions out of the total placements.
The Submission status column shows the number of forms submitted v/s the number received.
The Action column has the appropriate redistribute / Send Reminder buttons enabled as applicable.

Distributing Form / Evaluation
- Select Courses from your dashboard or left menu.
- On the Course Offerings page, find and select the course you’d like to manage.
- Expand the Learning Activities option on the left panel and select Review.
- Click on Forms/Evaluations Distribution from the left panel.
- Select the forms you want to distribute, redistribute, or schedule for a future date.
- To distribute a single form, select the desired form and click 'Distribute' under the Action column.
- To distribute multiple forms, select the checkboxes next to the desired forms, then click 'Distribute' button above the forms table. If the form if set to Auto distribution, you can see Auto-Distribution icon under distribution status table.
- To distribute a single form, select the desired form and click 'Distribute' under the Action column.
- Select the forms you want to distribute, redistribute, or schedule for a future date.
- On the 'Distribute Form(s)' window, you'll see the name of the selected form. Under the 'Select Evaluators' section, choose the evaluator you wish to send the form to, then click 'Continue'.
- Select the recipient to whom you want to send the forms and click Distribute Now. Click filter icon
to filter the evaluator list.

- Check the recipient list and click next.
Note: If an override email address is specified, all email notifications will be sent to that address instead of the original recipient.
- In the Template section, the standard template appears by default. You can create your own template and optionally set it as the default.
- In the Preview section, you can send the email immediately by clicking the 'Send' button. If you prefer to schedule it, select the second option, 'Send email at a specific date and time,' enter the desired date and time, and then click 'Send'.
- In the Template section, you can perform the following:
